Before Your First Appointment

Before your first appointment, we request that you download and fill out this form. You can then either bring it in to your appointment or fax it to 585-434-4081. This form will give Dr. Tomaino both your background information and allow his staff to get a head start on the paperwork. This will help your first appointment move as smoothly as possible.

If you are visiting our office, you may park in either two locations listed below. The cost for each is $2.00.

Park in Portland Ave garage, take the elevator to the Hospital main entrance lobby and you will be directed to the Parnall Bldg a short walk away.
Pull up to the Main Hospital Entrance and use valet parking.

Please bring your parking ticket and ask us to validate it before you leave.

If you are concerned about whether or not your vehicle will fit in the parking garage, stop at the Valet; it is still $2 and it will be parked outdoors.

Your Visit

Our offices are open Monday through Friday, 8am � 5:30pm. Recognizing that you have a busy schedule, we also offer early morning and Saturday appointments upon request. We remain available at any time, even after hours for any shoulder, hand, or elbow injury requiring urgent attention. Simply call or email our office and we will arrange for prompt immediate service. We also understand that you time is valuable and will make a sincere effort to adhere to our appointment schedule, and appreciate your patience if we are late due to emergencies or hospital surgery.

HMO Referrals

If you are a patient covered by an HMO insurance plan, you must receive a referral from your primary care doctor prior to receiving treatment at Tomaino Orthopaedic Care for Shoulder, Hand and Elbow.

Prescription Refills

In order to process your request for prescription refills as quickly as possible, please notify your pharmacist or call our office. To ensure same day order fulfillment, please call before noon when possible. Calls received after 2:30 p.m. for routine refills will be handled on the next business day.

To complete your prescription order, please have the following information ready when you call:

Patient Info: name, address and date of birth, cell phone, email address
Medication name
Allergies
Pharmacy: name and phone number

During evenings, weekends, and holidays, the doctor on call cannot refill routine prescriptions. Please contact the office during weekdays and before your medication has completely run out. We do not call in pain medication narcotics over the weekend or after 5 PM daily � instead make an appointment to see us or, if in an emergency, call to notify us that you are heading to the emergency room.

Payments and Insurance

We are members of most Preferred Provider Organizations and accept assignment from Medicare and Workers Compensation.

We make every effort to decrease the cost of your medical care. We request that payment arrangements be made for all office services at the time they are rendered unless prior arrangements have been made. We accept cash and most credit cards for convenience. We will bill your insurance company, however, payment is the patient's responsibility. We will assist you in handling claims in every way we can. Our main goal is to give you the appropriate medical care and our billing office will attempt to work with you regarding any special needs you may have.
